Athens is a huge metropolis with many different districts. From my quick trip, it looked like the Piraeus marina area was more upscale with fancier restaurants and shops, yet there was a row of small family restaurants that also lined the shore. In the downtown central area, the sidewalks were not always level and sometimes just abruptly stopped. Crosswalks were not always clearly marked or easily accessible. With that said, it was fairly flat walking around the central area of the city. If you plan to go to the Acropolis, the hike to the top is very strenuous (what I heard from fellow travelers). I'd advise a bus or other transportation to take you to the top if you have limited mobility.
